Annual Fee of $385 Includes:
- 2 Basic Dental Cleanings (Prophylaxis) each year
- 2 Periodic Exams per year
- 1 Set of Bitewing X-rays per year
- 1 Limited Exam and Periapical X-ray per year
- 1 Panoramic x-ray every 5 years
- 20% savings on all other dental work, no maximums (If paying with Care Credit, savings will be 10%)
**The cleanings included in the membership plan are strictly for preventive maintenance care. If patient presents with periodontal infection, a personalized treatment plan will be presented at an additional cost.
Terms & Conditions: This is not dental insurance. Cannot be combined with other insurance or discount plans. Membership plan is specific to one person and cannot be transferred. Membership term extends to 12 months from sign up date. No termination fee to cancel; however no refunds will be issued for the remainder of the year/months membership fees were paid. Services will be provided at this location only. Covered services not used within the specified time period will not be carried over or transferred.
Details & Limitations: Discount not applicable to dental products sold in the office. The cleanings included in the membership plan are strictly for preventive maintenance care. If patient presents with periodontal infection, a personalized treatment plan will be presented at an additional cost. If payment for restorative treatment is not provided at the time of services, then the 20% discount is void.
